Homeforts combines experienced support staff, specialist mental health expertise and dedicated dual diagnosis support with structured multidisciplinary oversight.
We hold weekly multidisciplinary team (MDT) meetings. Relevant Homeforts clinicians and professionals participate according to their roles and responsibilities. External NHS professionals and other members of an individual’s wider care team may contribute where appropriate and available. Attendance by every treating clinician is not a requirement of the MDT process.
