REFERRALS & ENQUIRIES

Let’s discuss
the right support.

We welcome suitable referrals from professionals, NHS services, local authorities, families and individuals.

WHO CAN REFER

A shared route
to specialist support.

We welcome referral enquiries from:

  • NHS organisations and integrated care boards.
  • Local authority commissioners and social workers.
  • NHS discharge teams and community mental health services.
  • Forensic mental health services.
  • Care coordinators and other healthcare professionals.
  • Families, carers and individuals seeking support.

SUITABILITY & ASSESSMENT

Understanding the person
and the support required.

Our referral process begins with an initial suitability review and, where appropriate, a comprehensive assessment of support needs, risks, staffing requirements and existing care arrangements.

We consider personal goals, preferences, clinical and social circumstances, safeguarding requirements, accommodation suitability and the level of support required.

All referrals, placements and support packages are subject to individual assessment, service suitability and available operational capacity.
