OUR SERVICE
Specialist Mental Health Supported Living.
Homeforts provides specialist, person-centred supported living services for adults aged 18 to 65 with complex mental health and associated support needs.
Our service combines practical day-to-day support, specialist mental health expertise, dedicated dual diagnosis support and weekly multidisciplinary reviews.
We do not replace existing NHS services or assume responsibility for specialist medical or psychiatric treatment. The individual’s treating clinicians retain responsibility for diagnosis, prescribing, medical treatment and specialist psychiatric management.
REFERRALS & SUPPORT
Individual assessment.
All referrals, placements and support packages are subject to individual assessment, service suitability and available operational capacity.
A general enquiry does not guarantee referral acceptance, an available placement, funding, a start date, a clinical appointment or a treatment outcome. Individual support arrangements are agreed through assessment and the relevant care and support process.
USING THE WEBSITE
